The following items are necessary prior to enrolling your child in kindergarten through 8th grade but we may require additional documentation or information.
Information needed:
Age Verification: original or certified copy of birth certificate (Mandatory for first time Kindergarten; if possible, for grades 1st-12th).
Immunization Records: Up-to-date immunization records including a Mantoux type TB test and required shots for age level
Physical Exam: Physical Exam for 1st grade only.
Information from Previous School: Check-out papers from previous school (not required but extremely helpful), Most recent IEP if student is in Special Education classes
